I give permission for my child named above to attend and participate in KIDS KLUB 2026 at Crosspointe Indian Church on the dates listed. I understand that activities may include Bible lessons, music, crafts, indoor games, snacks, and other age-appropriate events under adult supervision.
Medical Authorization
In the event of an emergency when I cannot be reached, I authorize the pastors, staff, or adult volunteers of Crosspointe Indian Church to obtain any necessary medical treatment for my child, including transport by emergency services if needed. I understand that reasonable efforts will be made to contact me first.
Liability Release
I understand that every effort will be made to ensure my child’s safety, but that participation in group activities involves some risk. I hereby release Crosspointe Indian Church, its pastors, staff, volunteers, and representatives from liability for ordinary injuries or illnesses that may occur during KIDS KLUB, except where prohibited by law, while not waiving any rights in cases of gross negligence or willful misconduct.
Pickup Authorization
I authorize the individuals in pickup section to pick up my child.
I understand that staff or volunteers may request identification from any person picking up my child, and that if no authorized person is available at pickup time, my child will remain under church supervision until I can be contacted.
